- Need to have or be eiligible for Medical Assistance - Need to get an assessement done through the county case manager or the public health nurse. - The care plan assessment includes going over the county assessment and organizing the information in a way that both caller and their PCA can follow. - The agency has qualified professionals that work with callers on this care plan in their home before services can be provided.

Description
PCA Choice is an alternative option for the traditional PCA Program in Minnesota allowing more control of services for the consumer. Services indclude in the Choice Option include: - Activities of Daily Living - Behavior Interventions: Includes observing and redirecting behavior that interferes with completion of PCA services. - Instrumental Activities of Daily Living: Meal planning, shopping, light housekeeping, participation in the community
