Eligibility

Children and adults living with developmental disabilities, or serious and persistent mental illness, or traumatic brain injuries that have a county case management referral, waivered services funding, county funding or private pay; serves residents of Anoka, Carver, Chisago, Hennepin, Ramsey, and Wright counties

Referral completed by county case management, and support plan medical history physician orders

- Office: 8 am - 5 pm Monday - Friday - Service available: 24 hours/7 days a week

Corporate Foster Care: Provides corporate foster care to children and adults with serious and persistent mental illness, and individuals with traumatic brain injuries. Programs are based in residential neighborhoods, usually with four individuals of similar needs and interests living together. Provides a living environment that promotes as much independence as possible, within a supervised setting. Training is provided in a range of areas, including environmental and behavioral strategies, daily living activities, and independent living skills. Supported Living Services: Provides supported living services to children and adults with developmental disabilities. Programs are based in residential neighborhoods, usually with four individuals of similar needs and interests living together. Provides a supported living environment that promotes as much independence as possible, within a supervised setting. Training is provided in a range of areas, including environmental and behavioral strategies, plus daily living activities such as social skills, community integration, and recreational and leisure activities. Provides independent living skills instruction such as money management, domestic tasks, transportation training, personal hygiene, program solving skills, among others.
