Application process

- Visit website for application/registration form. After the application is received, a letter is sent to the applicant within 10-14 days by mail notifying them if they qualify for the program or not. If they qualify, the amount the bill is discounted is included in the letter. The program qualification lasts three months from the approval date. Individuals need to apply again after three months to see if they qualify for the program. - Mail completed application to PO Box 183 Minneapolis, MN 55480-0183 - Email completed application to [email protected] - Walk-in to clinic to submit an application. Put application and required documents in a sealed envelope labeled "Patient Accounting" to ensure confidentiality.

Description

Patients who need to speak with a financial counselor, get cost-of-care estimates, or determine if they may qualify for financial assistance to pay for procedures may reach out about options. Financial counselors can help individuals understand patients' responsibilities and may offer no-interest payment plans. The program cannot be used for dental and pharmacy bills and charges.
