Minnesota Family Planning Program - Minnesota Department of Human Services
The Minnesota Family Planning Program is a health care program that covers family planning services (including related supplies) and transportation services to and from providers of family planning services. Family Planning Services covered may include: - Diagnosis and treatment of sexually transmitted diseases found during a family planning visit - Family planning office visits and education - Sterilizations - Transportation to and from a provider of family planning services - Various birth control methods NOTE: This program does not pay for abortions

Application process
Call; visit website to download an application or get an application from a certified Minnesota Health Care Program and mail it to DHS at: Minnesota Family Planning Program PO Box 64960 Saint Paul, MN, 55164-0960 Short-term coverage may be granted by the certified MFPP providers. Final eligibility is determined by the state. Coverage lasts one year.

Individuals 18 and older who meet the following criteria: - Cannot be pregnant - Cannot be enrolled in Medical Assistance - Must be a Minnesota resident - Must be a U.S. citizen or a qualified noncitizen - Must meet the income limits based on family size
