Recreation Fee Assistance Program - Minneapolis Park and Recreation Board

Required documents
Proof of Minneapolis residency and:
  • A copy of the two most recent pay stubs of all income earners in the household with Social Security numbers blocked out
  • A copy of previous year's federal tax returns with Social Security numbers blocked out. If there is no income and income taxes are not filed, attach a brief note explaining that there is no income
