Eligibility
- Adults ages 18 and older who have developmental disabilities and are residents of Wabasha County
- Individuals must receive or be eligible for one of the following funding/service options:
- Home and Community-Based Services (HCBS) through Medical Assistance (MA) or a county contract;
- A Home and Community-Based Services waiver, such as the Developmental Disabilities (DD) Waiver; or
- Services through an Intermediate Care Facility for Persons with Developmental Disabilities (ICF/DD)

Service area
Provides individualized day support, employment, and habilitation services for adults with intellectual and developmental disabilities, including:
- Adult Day Program: Provides day support, functional skill training, and activities designed to increase independence, community participation, and quality of life.
- Employment Exploration Services: Helps individuals explore employment interests, learn about work opportunities, and make informed choices about competitive integrated employment.
- Employment Development Services: Assists individuals with identifying employment goals, developing job skills, and preparing for competitive employment opportunities.
- Employment Support Services: Provides ongoing support, job coaching, and assistance to help individuals maintain employment in community-based or group employment settings.
- Exercise Therapy: Provides individualized exercise plans focused on mobility, physical activity, and functional skills to support overall wellness and independence.
- Habilitation Services: Provides individualized training and support to develop daily living skills, functional abilities, and community integration.
- Life Enrichment Services: Provides recreational, social, and community-based activities that promote personal interests, engagement, and quality of life.
- Vocational Services: Provides paid work opportunities through on-site production work and partnerships with area businesses.
