Eligibility

Primarily serves Lake of the Woods County To become licensed as an adult foster care provider the applicant must meet minimum licensing standards - Reside in the county in a home that meets fire- safety code; - Be at least 18 years of age; - Allow the county or state to complete a criminal history background check on all household members who are 13 years of age or older; - All household members must have a Physician's Statement that they are physically able to care for vulnerable adult(s) placed in the home; - Applicants must provide 3 positive references; - Applicants must have bedroom space available for resident(s) and have an approved fire inspection; - A completed homestudy of the family; - Meet required training hours annually.

Application process

Contact your county social services department and ask to speak with the licensing intake worker. Department of Human Services Application Form #0250 Call your county social service department and ask to speak with the licensing intake worker. Basic information about adult foster care licensing will be provided and any specific questions you have about adult foster care will be answered. Call or walk in.

Fees

None to apply for licensure, but a fee may be incurred with the criminal background check or State Fire Marshall inspection.

Description

?Adult foster care is a licensed living arrangement in a home that provides food, lodging, supervision and household services for qualified individuals. Also available is assistance with personal care and medication. Adult foster care providers a re primary caregivers for adults who cannot live alone. Caregivers are licensed by the Minnesota Department of Human services through Lake of the Woods County.
