Medical Emergency Fund - Bois Forte Band of Chippewa
Provides financial assistance for an individual to travel to care for or visit an immediate family member with a serious health condition that arises in emergency circumstances. The acute injury or illness must pose an immediate risk to a person's life or long-term health. Other instances where the fund may be accessed is a medically necessary surgery or 1 post-operation visit. A maximum of $400 in assistance is provided per patient per calendar year which includes: - $.21 per mile traveled - $70 per night lodging - $40 per day for meals

Application process
Visit website for application. Checks need to be picked up within 10 days from date of check or mailed to the applicant. All requests for same day services needs to be turned in by 2 pm to receive a check that day.

Eligibility
Applicants must be adults ages 18 and older who are a child, foster child, grandchild, grandparent, parent, significant other, spouse or stepchild of the patient. Patient or applicant must be a Bois Forte Band member. Patient or applicant must be a resident of Itasca, Koochiching or Saint Louis counties. Applicant must have to travel greater than 45 miles from home to the healthcare facility, exceptions to this rule include patients in the metropolitan area and/or patients who require an overnight stay.
