Housing and Community Support Programs - Rise, Inc.

Offers person-centered support services to individuals living with a disability or mental health condition. Services provided: - Case management to assess gaps in services to develop a goal plan to resolve the individual's housing crisis and prevent them from becoming homeless - Develop life skills including self-sufficiency planning, establish independence, and housing skills - Help individuals secure and maintain stable housing. Services include house searching, application assistance, budgeting, landlord communication, and ongoing stability planning. - Meeting with staff for information about housing resources for rapid re-housing, transitional housing, permanent supportive housing, and eligibility. Provides referrals and resources to programs as needed.
