Application process

A referral is required through the Coordinated Entry process. This process is open to individuals who are staying in an emergency shelter or working with a street outreach team and sleeping outdoors (camp, car, transit, etc). Individuals currently staying with friends or family, or who are in treatment will not be able to access Coordinated Entry.

Eligibility

- Must be 18 - 24 years of age - For singles, must be chronically homeless coming from streets or shelter - For young adult families, must be homeless coming from streets, shelter, transitional living or domestic violence situations - Must have HUD defined disability - Serves the Twin Cities metro area; residents of the program must be housed in Hennepin County
