Eligibility
Individuals age 18 and over who have been diagnosed with a mental health condition, are experiencing a substantial disability, and have impairment in three or more functional areas that prevent the individual from being self-sufficient, who are residents of Anoka, Dakota, Hennepin, and Ramsey counties

Recent diagnostic assessment from a mental health professional/clinic showing the need for ARMHS services proof of enrollment in a Minnesota Health Care Program (Medical Assistance/Medicaid (MA) card, PMAP, Minnesota Care)

Provides Adult Rehabilitative Mental Health Services (ARMHS) in four core service areas:
  • Basic Living and Social Skills: Offers one-to-one support to help participants build essential daily living and interpersonal skills. Skill areas may include cooking, nutrition, meal planning, budgeting, shopping, money management, time management, household organization, medication monitoring, wellness routines, and more.
  • Community Intervention: Services include assistance with accessing social services and community resources, addressing employment or education-related concerns, facilitating communication with agencies and families, and preventing hospitalization or higher levels of care. Helps resolve housing issues or landlord conflicts, promotes successful community reintegration, and supports crisis resolution and stability planning.
  • Medication Education: Services are customized to each client's treatment goals and needs, focusing on understanding the mental illness and related symptoms, educating on common and potential side effects of prescribed medications, and providing information on how medications help manage symptoms of mental illness.
  • Transition to Community Living: Assist individuals in transitioning from a higher level of mental health care back into the community by bridging the gap between discharge and independent living by coordinating care, promoting stability, and carrying out the discharge plan.
