Supports people in their own home or in their family home, placing a strong emphasis on skill building and community involvement through: - Accessing transportation and options for greater mobility - Budgeting, paying bills and gaining financial skills - Independently taking medication - Meal planning and preparation, including nutrition education - Obtaining and maintaining a home - Participating in the community in which an individual lives - Personal appearance and hygiene - Shopping Lutheran Social Services of Minnesota works closely with individuals, their families, legal representatives and support networks to develop a customized support plan based on their skills and interests.
