Individuals 60 years or older living in the Hamline Midway area (and part of the Frogtown Neighborhood) of St. Paul. Boundaries: University/Dale/Pierce Butler/Transfer Road

Application process
- Call or email. Callers may leave message. Free, 45 minute, in-office, in-home consultation, and/or phone support available, once or multiple times, but is not required to receive services. An appointment for consultation can be made within 1 - 2 business days. Services will be available within a couple days of the initial request or the consultation, possibly sooner depending on service.
- Transportation requests should be made two days in advance.
Fees
Volunteer coordination, rides, referrals and caregiver support are without charge. Nursing and home health aide services are $0 - $80+/hour based on ability to pay. Call for information.

Provides or arranges support for elder neighbors to assist them in living at home as long as possible. This support, which may otherwise not be available, uses the program's small staff, volunteers and/or paid nursing staff. Services include:
- Caregiver coaching and support groups
- Community Gardening
- Door-to-door transportation, both assisted and non-assisted, provided by volunteers. There is no charge, but donations are accepted. Accessible vehicles can be requested.
- Elders resource list of meal and grocery delivery, handyman services, modifications, etc. available by phone or Internet
- Exercise classes
- Friendly visiting
- Health education classes and events
- In-home eldercare assessment and resource recommendation
- In-home Internet/computer tutorial
- Linkage to county services
- Nursing referrals

- Respite care
- Seasonal raking and snow shoveling
