Optage Home and Community Services - Presbyterian Homes and Services

- In-home care including health care, therapies, homemaker and companion services. - House Calls providing primary care services by a physician or certified nurse practitioner. - Hospice care for end of life home care - Home delivered meals for the 7-county metro area are available as part of Optage services. Meals can also take place in a congregate setting at 17 locations throughout Hennepin, Ramsey, and Washington counties.

Application process

Call; email; or visit website for information Call to schedule an in-home assessment by a nurse which takes one to two hours. Client can request specific services, but still must participate in an assessment. The assessment can be arranged within 48 hours of receiving doctor's orders, or of the call if no doctor's orders are required. Services become available within a day or two after assessment depending on service needed.

Vary by service; $3 donation requested for meals
