Independent Living Program - Southeastern Minnesota Center For Independent Living - SEMCIL

Assists individuals living with a disability to identify goals and work to create a unique plan to achieve those goals. Services include: - Helps transition from facility to the community for individuals living in a nursing home or other group setting who wish to relocate back to their own home or community. Staff assist in exploring options, help obtain programs/services, and find the support needed at home. - Individual advocacy to understand rights, identify rights violations, and have tools to defend those rights. Advocacy topics include Individualized Education Plan (IEP), Social Security (SSI/SSDI) applications, and appeals for denied services - Provides a Chromebook and iPad Loan Program which allows individuals in need to loan a device for up to 60 days. Individuals who do not have internet access can also loan an internet service device. - Support planning to assist individuals with navigating options through Consumer Directed Community Supports (CDCS) plan
