- Individuals who are pregnant, have recently delivered a baby, or are parenting a child under the age of 4 1/2 - Families with children who have special pediatric services are eligible, such as NICU follow-up, Pediatric asthma, chronically ill children, and children with special needs due to lead poisoning - Residents of Mower County
Application/referral form birth certificate for families with children photo ID proof of residency (piece of mail, application of new ID, rental lease, bus ticket, etc.) shelter verification verification of the last 30 days of income and assets any other supporting documents can help process applications quickly
8 am - 4:30 pm Monday - Friday

Contact a doctor for a referral, or call the Public Nurse Office and ask to speak with the Maternal Child Health Coordinator
