Home visits are made to pregnant women identified as having an at risk pregnancy either because of medical factors such as a history of preterm labor or frequent miscarriages or because of social risk factors such as young age, lack of education, lack of resources. - Visits are supplemental to regular doctor exams - Client's physical condition is monitored - Referrals made to other appropriate community resources - At least one postpartum visit is made to new parents
