MnCHOICES provides a web-based application that is comprehensive and integrates assessment and support planning for people who need long-term services and supports.
NOTE: Community or in-person screening is required in some situations to determine eligibility for waiver services.
The assessment takes a person-centered approach to ensure services meet each individual's strengths, goals, preferences, and assessed needs; includes assessing what level of service is needed for care such as homemaker, home health aide, adult day care.
