- Visit website for application/registration form. - After the application is received, a letter is sent to the applicant within 10-14 days by mail notifying them if they qualify for the program or not. - If they qualify, the amount the bill is discounted is included in the letter. The program qualification lasts three months from the approval date. - Individuals need to apply again after three months to see if they qualify for the program.

Offers a discount program for eligible patients and a payment plan option for any balance not covered by the discount. The program cannot be used for dental and pharmacy bills and charges. Program assistance is based on: - Assets, such as property or savings accounts - Family size - Income - Liabilities, such as bills or regular payments due Individuals may apply even if they have insurance coverage.
