Adult Mental Health - Targeted Case Management - Northland Counseling Center - Hibbing
Targeted Case Management provides individualized services including: - Advocating for client rights: Includes civil commitment and/or court appointed services - Assistance in obtaining and coordinating needed services: Includes assistance with applying for Medical Assistance and/or other health insurance; applying for Social Security Disability and/or other sources of financial assistance - Encouraging successful community living: Connects to resources and/or supports with employment, education, housing, childcare, and transportation - Providing support: Includes assisting individuals and loved ones in understanding and coping with mental illness - Working closely with other community providers to ensure individual needs are met: Includes Case Managers assisting clients with direct access to all Northland Counseling Center services; connecting and/or referring to services in the area - Working with clients toward their recovery: Referrals and coordination with substance use disorder (SUD) services

Must have a diagnostic assessment completed within the past 6 months. Unless the client has Medical Assistance (MA) for health insurance, the referrals must go through Saint Louis County for County approval.
Eligibility
- Diagnostic assessment within the past 6 months with a diagnosis of an SPMI (Schizophrenia, Bipolar Disorder, Schizoaffective Disorder, Borderline Personality Disorder and/or Major Depressive Disorder); - Mental health hospitalization; or - Two or more contacts with mental health emergency services; and - Residents of northern Saint Louis County
