Application process
  • Call; visit online patient portal; visit website for financial assistance application form
  • Submit the financial assistance application form via email ([email protected]), fax, online patient portal, in person, or by mail (HealthPartners P.O. Box 773217 Detroit, MI 48277-3217)
  • Once all information is received, a letter about the application will be sent in 10-14 days
If the application is denied and there is an outstanding balance, payment arrangements can be made. If not future access to HealthPartners Clinics may be restricted. If one does not have insurance, they will be required to make a down payment before your appointment

Financial counselors can help individuals understand patients' responsibilities and may offer no interest payment plans. Other options may include:
  • Discount Program: Individuals who are uninsured or underinsured and have incomes less than 400% of the Federal Poverty Guideline may be eligible for help to pay their medical bill
