Walk-In Rule 25/Chemical Health Assessments - Park Avenue Center

The Park Avenue Center offers the Rule 25 on a walk-in and by appointment basis. The assessment lasts about an hour and includes questions about many different areas of a person's life. Role of the Rule 25: - First step in entering treatment - To assess a person's drug and/or alcohol use to determine if treatment is needed - To determine what level of care is most appropriate - To match a program that best meets the client's needs - Complete any insurance funding requirements. Whenever possible, the Assessor also speaks to a person that can also give information about the client being assessed. - Assessments meet court requirements

Application process

- Call for appointment; walk in's are first come, first serve - Appointments fill up fast and can only be taken up to 3 weeks in advance

Fee

- Hourly; free for those who qualify for Rule 25 funding or $100 for self-pay assessments that will meet court requirements - Clients with insurance, the assessment is typically covered, however, co-pays and deductibles do apply
