Case managers and social workers work collaboratively with individuals and families to address both immediate and longer-term areas of need.Provides the following:- Case Management: Assists with creating comprehensive, strengths-based case plans and individualized, support. Assists with safety planning, goal setting, and budgeting.- Culturally responsive, on-site, and community-based navigation services, and information and referrals to other vital community supports.- Rent Assistance Program: Short-term assistance for evictions or utility shut-offs for residents of Bloomington, Edina, Minneapolis, and Richfield; funding varies monthly and is very limited. NOTE: Check the website to see if rental applications are currently being accepted.
