Eligibility

Hennepin county residents who are 18 years of age or older, who have a persistent mental health illness, and who have 3 or more areas of life where independence and functioning need improvement

Required documents

Some form of ID and if insured, copy of insurance card, medical assistance card, medicare card, State ID proof of income in the form of check stubs, tax returns or letter from employer to apply for sliding scale fee.

Call; must have mental health assessment within 6-month period; visit website for referral form

Fees

- Call for fee information; Paid by insurance; Sliding scale - Patients who are uninsured and reside in Hennepin County may be eligible for discounted services. Clinic is able to set up a payment plan if requested. A 5% discount is given if the bill is paid in full on same day as visit. (Some restrictions apply to certain services.)
