- Individuals living with a physical or developmental disability, medical condition, have a suitable waiver, or can privately pay - Residents of Anoka, Dakota, Hennepin, Ramsey, and Washington counties
Birth certificate health insurance card Medical Assistance/Medicaid (MA) card Medical history Medicare card physician orders Social Security card/number. Medical history and physician orders are needed to help residents meet their medical needs.
- Office: 8:30 am - 5 pm Monday - Friday - 24 hours/7 days a week service at group homes
American Sign Language (ASL) available at some sites
- Call; email; visit website - Call to arrange meeting with Director of Development. Interviews will take place with the person requiring service and/or persons who know the individual best. The individual will meet housemates and tour the home to see if he or she is interested in pursuing the placement. Information about the care required will be gathered and evaluated. If appropriate for an opening, negotiations for funding are made with the financially responsible party (county, U-Care, or individual). - Once approved, move-in can occur within a couple of weeks.

Operates over 40 group homes located throughout the five-county suburban Twin Cities. Services provided: - 24-hour nursing services - Have live-in staff in each of the homes to lend continuity and support as needed - Respite care services to individuals with hospital level care - Support for individuals living with a mobility and/or medical need that require care of two-person transfers, special diets, positioning, range of motion therapies, tube feedings, ostomy care, supplemental oxygen, and diabetes
