Eligibility
- Individuals who are pregnant, have recently delivered a baby, or are parenting a child under the age of 4 1/2
- Families with children who have special pediatric services are eligible, such as NICU follow-up, Pediatric asthma, chronically ill children, and children with special needs due to lead poisoning
- Residents of Nicollet County
