Follow Along Program - Hennepin County Human Services - Public Health

Developmental tracking for children ages birth to age 3 through a series of age appropriate questionnaires for parents or caregivers. Services include: - Periodic developmental tracking - Age-appropriate activity sheets for child - Parent education information - Information and referral in accessing needed services NOTE: Children receiving Early Childhood Special Education services do not qualify for the Follow Along Program as it would be a duplication of services

- Call for more information and to enroll. Staff will mail out forms to be filled out with a return envelope. - Participation is voluntary. Participants can leave the program and re-enter at any time. - Visit the Minnesota Department of Health's website to enroll online at https://cfhp.health.state.mn.us/fap/ - Families living in the cities of Bloomington, Eden Prairie, Edina and Richfield should contact April Sommerville, [email protected] or call separate intake phone

Required documents

Application/registration form; birth certificate for families with children; photo ID; proof of residency (piece of mail, application of new ID, rental lease, bus ticket, etc.); shelter verification; verification of the last 30 days of income and assets; any supporting documents can help process applications quickly
