Car Seat Program - Countryside Public Health Department

Eligibility

  • Primarily based on insurance coverage (Medical Assistance, UCare, Blue Plus) or demonstrated financial need
  • Additional eligibility may be determined through clinical referral or safety assessment
  • Parents or guardians must complete required education and be present for installation

Child's age or due date proof of insurance (if applicable) safety agreement/participation form

8 am - 4:30 pm Monday - Friday

Interpreters available; Spanish

  • Attendance at car seat education session

  • Insurance will be billed if applicable
  • Also offers a grant program to provide car seats at no cost for those in need
