Street Outreach/Crisis Line

Application process

- Call or walk-in to a drop-in center or shelter to connect with a street outreach worker for a Coordinated Entry Assessment - A referral is required through the Hennepin County Coordinated Entry process. This process is open to individuals who are staying in an emergency shelter or working with a street outreach team and sleeping outdoors (camp, car, transit, etc). Individuals currently staying with friends or family, or who are in treatment, will not be able to access Coordinated Entry.
