Eligibility

Meet the following criteria: - Must have a household income that is at or below 50% of the Area Median Income as determined by HUD - Must own the home or make mortgage payments on the home for which they are seeking assistance and be up to date on tax payments - Must plan to remain in their home for at least two years - Someone in the home must be 55 years or older and/or living with a disability - Serves Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, and Washington counties in the Twin Cities, and Cottonwood, Jackson, Murray, Nobles, Pipestone, and Rock counties in Southwest Minnesota

Application form homeowner's association information if applicable household income name, date of birth, and demographic information for all occupants of the house if veteran, service dates and branch of service

Provides minor home safety and fall prevention modifications and ramps at no cost to homeowners in need. Repair services will allow homeowners to remain independent and live safely in their homes for a longer period of time. Types of repairs may include grab bars, handrails, wheelchair ramps, and shower seats.
