Fee

People with incomes, earned and unearned, equal to or above 100 percent of the Federal Poverty Guidelines (FPG) must pay a premium based on a sliding fee scale. There is no maximum income limit or maximum premium amount.

Eligibility

Primarily serves Koochiching County. Be certified disabled by either the Social Security Administration or the State Medical Review Team. Be at least 16 but under 65 years of age. Be employed. Not be eligible for Medical Assistance under other, more beneficial categories. Meet the asset limit of $20,000. Pay a premium, if required.
