Older adults (55 years and older) in Olmsted County that are navigating housing transition
Required documents
Client intake is completed to assess basic contact details, health information, emergency contact, current services and supports the client is receiving, their goals for housing transition, and their current housing transition needs.

9 am - 1 pm Friday by appointment

HOME Transition Mentorship Pilot Program:- Provides mentorship for older adults to support their goals to transition to the right home that fits their needs and enables them to live and thrive. -Provides guidance and support to Olmsted County older adults interested in making a housing transition. -Through mentorship support, helps older adults identify what is the right next "home" for them, and makes an actionable plan to reach their goals. -Provides education to older adults about housing needs and resources as they age.
