Eligibility

  • Serves individuals with a terminal illness and a life expectancy of approximately six months or less, as certified by a physician- Admission to its inpatient hospice cottage is generally intended for individuals who are unable to safely remain in their current living environment and do not have adequate caregiver support at home.- Services are available to residents of Cottonwood, Lyon, Murray, Nobles, Pipestone, and Redwood counties

Provides an inpatient hospice cottage offering 24-hour care in a home-like setting under the supervision of a Registered Nurse and hospice care team. Services focus on comfort, dignity, and family support during end-of-life care.Services include:- 24-hour nursing oversight with hospice-trained staff providing comfort care and assistance- A comfortable setting that allows family members and loved ones to spend time with the patient during end-of-life care- Bereavement support for family members for up to one year following the patient's death- Education and guidance for family members on caregiving and end-of-life support- Meals, snacks, laundry, and assistance with personal care (such as bathing, dressing, and hygiene)- Medications related to hospice comfort care provided in accordance with hospice benefit guidelines- Volunteer support including companionship, respite visits, help with errands, and light household assistance
