Children and adults who have completed a diagnostic assessment, or assessment update within the last 6 months and were diagnosed with a serious and persistent mental illness or severe emotional disturbance, and are a resident of Grant and Pope County

- Medical Assistance
Provides targeted mental health case management services to address the unique needs of an adult or child who is experiencing mental health concerns to help coordinate and implement mental health services. Case managers can also assist in referrals to employment support and housing programs.
