Description

Provides assistance to older adults to help them remain in their homes in the healthiest and safest manner possible. The Outreach Worker meets at the client's home to identify needs, discussing with the client appropriate programs or services. Clients are linked to services including home delivered meals, food programs (SNAP), economic assistance, energy assistance, chore services, etc. The Outreach Worker completes the necessary paperwork for various services.
