Eligibility

Persons who live within a 40 mile radius and require part-time skilled care who are residents of Mille Lacs County and parts of Aitkin, Crow Wing, Kanabec, Morrison, and Pine counties Hospice Care: Must have a terminal diagnosis of six (6) months or less life expectancy, as certified by a physician and no longer seeing or receiving curative treatment
