Must know medication needed and complete the online application process to determine eligibility. - Provides access to co-pay and savings offers for a variety of brand-name medicines NOTE: Once forms are filled out for the Pfizer Patient Assistance Program, the patient and their healthcare provider must print their portion of the form and work together to complete the hard copy form and submit it to the program. The Pfizer Patient Assistance Program is not health insurance.

Individuals residing in Minnesota who meet at or below 300% of the federal poverty guideline and must: - Have a valid prescription for the Pfizer medicine they are seeking - Have no prescription coverage or are unable to afford coverage - Be treated by a healthcare provider licensed in the U.S. or a U.S. territory
