Eligibility

An assessment is required to access any substance use disorder services. The assessment will determine individuals diagnosis and level of care recommended. Serves residents of Anoka, Carver, Dakota, Hennepin, Ramsey, Scott, and Washington counties

Referral is required unless in-house assessment is conducted.

Description

Rule 25 and comprehensive assessments provides an appraisal for substance effects on an individual and their family and creates an individual program to deal with those effects. Create also refers clients to treatment for substance abuse as well as to services dealing with underlying problems. Service is provided both in-person and virtually through Telehealth link.
