Provides mental health services such as: - Case management services - Mental health assessment for referrals to community mental health centers, outpatient treatment, residential treatment facilities, board and lodges, therapy referrals, and other resources - Prepetition screening for all individuals petitioned for civil commitments for a mental illness due to being a danger to themselves or others NOTE: Mental health crisis services are provided by another agency named Alluma for crisis assistance and intervention

Eligibility

Adults who are diagnosed with a mental illness or emotional disturbance, and are a resident of Kittson County. Must also meet one of the criteria:

- Has had at least two episodes of inpatient care for mental illness in the last 24 months

- Has had continuous psychiatric hospitalization or residential treatment of at least six months in the previous 12 months

- Have a written opinion from a mental health professional that case management is needed to prevent future hospitalization

- The individual was eligible for case management services as a child
