Description

- Waiver and Community Support Grand Information (call for full requirements of each waiver) - Alternative Care: For those 65 or over who are NOT eligible for Medical Assistance -Community Alternative Care: For those under 65, certified as disabled, eligible for MA and need care at the level provided in a hospital -Community Alternatives for Disabled Individuals: For those under age 65, with a certified disability, who are eligible for MA and need services to remain at home. -Community Support Grand: For those eligible for MA are able to direct and purchase their own care and supports, have functional limitations that require community support, live in a natural home setting, are eligible to receive home care services from MA home care agency, and not participating in the Alternative Care Program -Developmentally Disabled Waiver: For those who are eligible for MA based on disability diagnosis, have a developmental disability, meet the ICF/DD level of care criteria, require daily interventions, assessed to need a residential habilitation service, have made an informed choice of waiver services instead of ICF/DD services, and have an assessed need for supports and services over and above those available through the MA state plan -Elderly Waiver: For those who are at least 65, eligible for MA, and need services to be able to stay at home -Traumatic Brain Injury: For those under 65 who have documented diagnosis of traumatic or acquired brain injury, are certified disabled, are eligible for MA, and need care at the level provided in a nursing home or hospital
