A service of the Minnesota Board on Aging in partnership with Minnesota's Area Agencies on Aging. Services include: - Community Supports: Connects people with community supports, such as home health care, meal preparation/delivery, transportation, and hiring someone to help at home or with yard work - Health Insurance Counseling: Helps people with questions about Medicare and other health care questions - Housing Resources: Helps people understand their housing options, such as assisted living requirements, comparing housing services and costs, and more - Preadmission Screening (PAS): Manages preadmission screening (PAS) for anyone planning to be admitted into a Minnesota nursing home - Prescription Drug Assistance: Helps those who qualify find ways to reduce prescription drug costs, including finding manufacturers' patient assistance programs and completing applications - Website of Resources: Offers a website to locate resources throughout the state for help with the home, medical costs, tax preparation, legal/advocacy, housing, transportation, food programs, benefits, health care, mental health, and more. NOTE: Older adults who have a case manager or care coordinator or may have questions about county program and eligibility should be referred elsewhere. The individual should be referred to their case manager, care coordinator, or county agency.
