Nurse home visiting to assess, monitor and educate parents with young children Home visits to provide information for the following: - To promote healthy pregnancies - Nutrition - Labor and delivery - Breastfeeding - Community resources/referral New baby home visits: - Support and information following delivery for parents and baby - Health promotion parent/child/family home visiting - Provide age appropriate information and resources for the growing child - Encourage positive parenting - Home safety checks - Early intervention for young children - Coordination of services for children with disabilities/special needs and their families
