Description

Housing Stabilization Service - Provides support to individuals and families in securing and maintaining safe, stable housing. Works closely with various providers and shelters to ensure that everyone has access to the housing resources. Housing Stabilization Support - Personalized Housing Plans: Collaborates with clients to develop tailored housing plans that address needs and goals - Ongoing Case Management: Continuous support to navigate housing challenges, including financial management, lease agreements, and landlord relations - Crisis Intervention: Immediate assistance during housing emergencies to prevent homelessness and ensure safety Connection with Providers and Shelters - Comprehensive Network: Partners with local shelters and housing providers to connect clients with available resources - Referral Services: Streamlined referrals to emergency shelters, transitional housing, and permanent supportive housing - Collaboration with Community Partners: Works with community organizations to maximize support and resources for clients Supportive Housing - Assists individuals experiencing homelessness by exploring housing options, access funding to obtain housing, and transition to housing 24-Hour Emergency Assistance - Provides on-call counseling and problem solving and/or immediate response for assistance at a person's home due to a health or personal emergency. NOTE: Program does not provide direct rental assistance. Only helps navigate resources to access funding.
