- Names, address, birth dates of everyone living in household- All insurance cards for members of household who want to become patients- Proof of income which could include pay stubs, a bank statement, a benefit check, tax return, unemployment or disability payment or Household Assessment is required in order to receive CUHCC sliding scale fee- Picture ID

Medical Services:- Adult and pediatric preventative and acute care services- Adolescent and teen clinics- Diabetes education groups in multiple languages- Family planning- Obstetrical women's clinics and midwifery- Lead screening- Medical case management services- Routine adult and child immunizations, and travel immunizations- Provides flu shots- Sexually transmitted disease testing and treatment- Testing for sickle cell anemia and other hemoglobinopathies- Screening for breast and cervical cancersDental Services:- Cleanings, x-rays, and exams- Crowns and bridges- Deep cleaning and instruction in oral hygiene- Fillings- Root canals- Sealants, flouride treatments, and education- Tooth extractions
