Description

Provides Family Homeless Prevention and Assistance Program (FHPAP) services and is accessible to individuals who are facing a housing crisis, are homeless, or are at risk of imminent homelessness. Services include: - Prevention Services: Temporary financial assistance and/or supportive services to help people remain in their current housing situation, relocate, or divert them from entering the homeless system. Financial assistance is provided for rental assistance, mortgage assistance, and utility payment assistance. - Street Outreach: Link individuals who are homeless to available resources such as shelter, housing, and/or critical health and supportive services - Rapid-Rehousing: Assist households to quickly exit homelessness and return to permanent housing. Provides short-to medium-term rental assistance and supportive services such as case management and housing navigation.
