Application process

To apply, individuals and families should follow these steps: 1. Visit the website https://Renthelphennepin.hdsallita.com to determine if they qualify 2. Once they determine that they qualify, mail or call the Sabathani office to get a list of necessary forms 3. Gather and submit the following documents: W9 from from property manager; ledger from property manager showing the amount owed as of the current day's date; proof of income from the last 60 days (pay stubs, benefit letters, etc.); current lease (signed copy); eviction notice; denial letter from Emergency Assistance

Eligibility

Individuals and families who meet below 200% of the Federal Poverty Guideline and are residents of Hennepin County, preferably near the 38th Street corridor
