Home Health Care and Hospice - New Ulm Medical Center

Home Health Care: Experienced nurse case managers visits each patient's home. Nurses provide assessment, teaching, medication management and skilled nursing procedures while evaluating progress. The nurse case manager coordinates care with the patient's physician, family and other health care professionals. Other services can include home health aide visits, social services, and professional therapies such as physical, occupational, and speech therapy. These services will vary depending on your doctor's order. Hospice Services: Hospice care is for anyone with a life-limiting illness and whose life expectancy is determined to be six months or less if the disease runs its natural course. Our team focuses on enhancing quality of life wherever terminally ill patients call home, through expert pain and symptom care along with emotional, social and spiritual support.
