Eligibility
- Adults must be ages 18 or older, diagnosed with a severe mental illness and suffer from functional impairments, and have insurance coverage of Medical Assistance or another plan with ARMHS services coverage
- Residents of Anoka, Chisago, Dakota, Hennepin, Isanti, Ramsey, and Washington counties
