Care Consultation for Families, Professionals and Individuals With Early Dementia - Alzheimer's Association Minnesota - North Dakota Chapter

Care consultations consist of individualized problem solving, provision of support and assistance with resources. Meetings will assist in identifying needs, creating action plans and identifying appropriate resources and may be done by phone or in person and are available to persons with dementia and their family and professional care partners.

Eligibility

Open to all in Minnesota and North Dakota experiencing Alzheimer's disease, a related dementia, or memory loss in themselves or a family member. Professionals may also use the Information Helpline and Care Consultation for assistance with dementia/memory loss related issues.
