Persons age 45 and older who require assistance in their homes; primarily serves residents of Dakota, Chisago, Hennepin, Isanti, Ramsey, Scott, Washington and Wright counties. Other areas considered.

Required documents

A doctor's order is not necessary to provide live-in home health aide assistance medication monitoring administration does require attending physician orders

Office hours: 8 am - 5 pm Monday - Friday care provided 24 hours/7 days a week

Application process

Call; walk in; email; visit website; RN will provide an assessment to determine services. The two-hour assessment is done in the client's home.
