Family Healing Programming: Open to all youth ages six and older. Specific program eligibility includes:
- Male Programming: Open to all male youth ages 10 -18
- New Trails Group Home: Open to all male youth ages 14 - 19
- Onamia Academy: Open to all grades 4 - 12
- Willow Trails Group Home: Open to all female youth ages 12 - 18

Provides the following services:
Emotional, Behavioral, and Mental Health Program: Serves youth who experience challenges with emotions, behavior, and mental health. The program provides structured, therapeutic support to help youth heal, grow, and develop the skills needed to thrive. Staff are highly experienced in supporting youth with complex emotional and behavioral needs.
Intellectually and Developmentally Delayed and Special Needs Program: Supports youth with intellectual or developmental delays and special needs. A safe, structured environment allows youth to grow, build skills, and make progress at their own pace while receiving individualized support tailored to their unique needs.
New Trails Group Home: Supports male youth transitioning from residential treatment into a more independent setting. The program offers 24-hour staff supervision, individualized coaching, and mentoring. Youth practice and implement positive life skills in a safe and supportive environment as part of their transition toward independence. Serves youth ages 14-19 years old.
Onamia Academy: Provides an on-campus school serving youth receiving treatment at Nexus-Mille Lacs Family Healing. The school serves students in grades 4 through 12 and focuses on meeting each student's individual educational needs. Small class sizes and tailored education plans support academic progress, graduation requirements, and diploma standards.
- When an Individualized Education Program (IEP) identifies a need for additional educational support, the academic resource room provides enhanced guidance to meet diverse learning needs. The school is operated by the Onamia Public School System of Minnesota, and graduation and credit requirements are governed by the local school district.
