Family-Centered Residential Treatment for Youth - Nexus Family Healing

Provides the following services: Emotional, Behavioral, and Mental Health Program: Serves youth who experience challenges with emotions, behavior, and mental health. The program provides structured, therapeutic support to help youth heal, grow, and develop the skills needed to thrive. Staff are highly experienced in supporting youth with complex emotional and behavioral needs. Intellectually and Developmentally Delayed and Special Needs Program: Supports youth with intellectual or developmental delays and special needs. A safe, structured environment allows youth to grow, build skills, and make progress at their own pace while receiving individualized support tailored to their unique needs. New Trails Group Home: Supports male youth transitioning from residential treatment into a more independent setting. The program offers 24-hour staff supervision, individualized coaching, and mentoring. Youth practice and implement positive life skills in a safe and supportive environment as part of their transition toward independence. Serves youth ages 14-19 years old. Onamia Academy: Provides an on-campus school serving youth receiving treatment at Nexus-Mille Lacs Family Healing. The school serves students in grades 4 through 12 and focuses on meeting each student's individual educational needs. Small class sizes and tailored education plans support academic progress, graduation requirements, and diploma standards.

• When an Individualized Education Program (IEP) identifies a need for additional educational support, the academic resource room provides enhanced guidance to meet diverse learning needs. The school is operated by the Onamia Public School System of Minnesota, and graduation and credit requirements are governed by the local school district.

Programs for Males: Serves youth ages 10 to 18, with age ranges varying by program. These programs provide developmentally appropriate treatment, education, and support designed to promote healing, accountability, and long-term success. School and Vocational Training: Provides support to youth in developing academic achievement and practical life skills. Education and hands-on learning opportunities prepare youth for future employment and independence. Sexually Problematic Behaviors Program: Specializes in treating youth who demonstrate sexually problematic behaviors. Treatment is grounded in current research and national standards and focuses on the whole child rather than a single behavior. The program is committed to breaking cycles of sexual harm and supporting healing, accountability, and healthy development. Vocational Opportunities: Provides youth with hands-on experiences to build job readiness and life skills. Youth participate in structured activities that promote responsibility, teamwork, and community engagement, including:

• Kids CARE work-opportunity program

• Woodworking

Willow Trails Group Home: Provides female youth with a safe, structured, and supportive living environment. The program fosters personal growth through strong, supportive relationships with staff. Youth receive guidance in emotional healing, character development, and social maturity while addressing the underlying trauma and mental health needs that contributed to placement. Positive and effective behavior techniques are used to promote healing and stability, creating a place youth can call home. Serves female youth ages 12 - 18 years old.
